The following forms are provided to assist traditional Educator Preparation Providers. Providers are
encouraged to contact the South Carolina Department of Education (SCDE) staff prior to submissions.
 

New or Modified Programs

• Statement of Intent Form for New or Modified Programs - Public & Private Institutions
  Completed by educator preparation providers to offer new or modified programs or endorsements. The Statement of Intent Form should be submitted a minimum of 60 days before the full program proposal will be submitted to SCDE staff.
• New or Modified Program Proposal Requirements - Public & Private Institutions
  Completed by public and private institutions of higher education to propose new programs or make modifications to existing programs. An approved South Carolina Commission on Higher Education proposal is submitted by public institutions in lieu of sections I and II of the proposal. All submissions should be made accessible for those with disabilities prior to submission. A Statement of Intent Form should be submitted prior to the New or Modified Program Review proposal.

Progress Monitoring Report - Accreditation

• Progress Monitoring Report and Instructions
  Completed by institutions of higher education that have been granted probationary status through either the national or state accreditation review process to provide status updates to the State Board of Education.

Recommendation for Certification

• College Recommendation Form - Classroom Teaching Fields
  Completed by institutions of higher education to verify completion of an approved teacher preparation program. 
• College Recommendation Form - Service & Leadership Fields
  Completed by institutions of higher education to verify completion of an approved, advanced preparation program leading to certification in a service or leadership field.

Read to Succeed (R2S)

• Read to Succeed Pre-Service Verification Form
  Completed by institutions of higher education to submit Read to Succeed course offerings and changes. The Office of Educator Services verifies course compliance with the Read to Succeed statute.
• Read to Succeed Course Substitution Form
  Completed by institutions of higher education to submit Read to Succeed course substitution requests. The Office of Educator Services verifies course compliance with the Read to Succeed statute.
