| Employees enrolled in a health plan automatically receive Basic Long Term Disability Insurance at no charge. Benefits are limited to a maximum of $800 per month. This insurance is provided at no cost to the employee. You may purchase Supplemental LongTerm Disability Insurance to protect a greater portion of your income should you become disabled. |

Supplemental Long Term Disability Insurance

| In addition to the state-provided disability insurance, employees may purchase this supplemental insurance to protect a greater portion of their income should they become disabled. The maximum monthly benefit is $8,000 with a guaranteed monthly benefit of at least $100. Premiums are based on the employees age and choice of plan. |

Long Term Care Insurance is designed to help employees protect their financial resources from the expenses of long term care. You can purchase this insurance for yourself, your spouse, your parents and your spouse’s parents. Premiums are based on age at enrollment and the benefit level selected. |
