This form is completed by a college to verify that you completed an approved educator preparation program leading to educator certification in a given state and to recommend you for a given classroom certification area(s).  Please be aware there is a seperate form for service and leadership recommendations.

Colleges and Universities complete this form to verify completion of an advanced program leading to certification in a service or leadership field of educator certification.

This form is completed by the dean or dean's designee of a regionally accredited institution of higher education to verify that the content of a course completed over seven years prior has not significantly changed from current course content offerings.  Revalidation is only for use in meeting BA+18 or MA+30 class level upgrade requirements.

This form may be completed by another state's Department of Education branch to verify an educator's credential history.

This form is used to request that the Office of Educator Services evaluate prior work experience as an educator for inclusion on an educator certification.

Pursuant to S.C. Code Ann. § 59-25-60, as established by Act 29 of 2025–26, the South Carolina Department of Education (SCDE) is authorized to award experience credit for eligible non-teaching occupational experience directly related to the certification content area for which an educator qualifies. The State Board of Education approved implementation guidelines on November 4, 2025. 

This form is completed by the employing South Carolina public school district in order to fill employment opportunities with international applicants.
